Physical Information

All students must have a physical and all other necessary forms on file on the Final Forms website prior to tryouts. Athletes will not be allowed to tryout if the physical and accompanying forms have not been turned submitted digitally.

Student athletes wishing to participate in CNMS athletics (summer, fall, winter, or spring) will need to have an IHSAA physical submitted, signed by a physician, to Final Forms that is dated on or after April 1^st of 2025. ALL online forms can be completed online on the Final Forms website.  In addition, you will need to upload a photo or a scan of your updated IHSAA Physical on the same site. 

Need an athletic physical completed quickly? You can get one at Mercy Urgent Care in Newburgh for $20 (subject to change). Walk-ins are accepted.

ITEMS NEEDED TO PARTICIPATE IN ATHLETICS AT CASTLE NORTH MIDDLE SCHOOL (ALL items can be completed/uploaded on Final Forms—PLEASE DO NOT TURN IN PHYSICAL FORMS TO THE OFFICE OR A MEMBER OF THE CNMS COACHING STAFF!!) :

2. Health History & Medical Profile (Medications, Injuries & Hospitalizations, Past & Ongoing Conditions)
3. IHSAA PPE Physician Questions (to be completed prior to physical examination)
4. Consent for Medical Care
5. Transportation Release
6. Athletic Code of Conduct
7. ProRehab Consent and Release
8. Concussion & Cardiac Arrest Awareness
9. IHSAA PPE Physical (this will be uploaded to Final Forms)
